An employee was operating a riding pallet jack, staging a load of pallets. As he was backing the pallets into the staging area, the top pallet came in contact with a concrete pillar. It shifted forward and struck the employee's right hand as it rested on the throttle control of the pallet jack. The employee suffered a compound fracture and laceration to the little finger, resulting in a medical amputation to the finger.

Two employees were removing a screw from a high-speed injection molding machine. As they were removing the screw tip, pressurized molten plastic was released through an opening around the screw tip which resulted in both employees being burned by the molten plastic. They sustained burns to the forehead, right neck, face, right arm, and right hand. One employee was hospitalized.

An employee was using a come-a-long to raise a basket out of a sump to clean it. When the basket reached the top, the come-a-long slipped out of gear and the basket fell. As it fell, the basket pulled the employee's left index finger into the come-a-long, resulting in an amputation of the left index finger up to the first knuckle.
